Don't recognize it right off but a good hardware store like an Ace can probably match it up.

I found my exact faucet assembly on Amazon except is was chrome instead of bronze. It kept leaking even after changing out the cartridges. Save your money and replace the whole thing now.
I ordered it and are very happy with it. Having the long neck helps when washing big pots and pans in my small sink.

thanks all! New faucet it shall be.....I wanted somthing with ceramic cartridge technology anyway.....
