This is a pretty sad experience.  Seems did not have enough truck.

While his F250 didn't have near the payload to handle that 16k+ fifth wheel, especially with those aftermarket wheels and lift kit, it seems to me the fifth wheel hitch wasn't installed properly and is what more than likely caused the accident.  I suppose, too bad there isn't video of the crash actually happening, that would have been more entertaining.
 
Agree re the hitch install. Have seen where bed rails were only attached to the truck bed and not the frame or bolts/nuts not torqued properly. Looks like the rear rail came loose.
