the original story had him "injured" not deceased (story was updated) but having said that even what sort of mechanical issue Brakes? With the number of over loaded units i see everyday and i drive 60,000 miles a year or more for business plus another 10-15,000 for pleasure pulling my 5er "Crazies" may not be a wrong term - however my sympathies to the innocent victims
 
Well even overload.  I don't see evidence of that (Oh perhaps a couple hundred pounds but well within "Wiggle room" I can think of two failures that miight cause that. Throttle return spring. not all  know how to deal with that. and brake failure. again there is a way to deal with that. if you think fast enough. Darn few do,  Since they said "Mechanical failure" that means it was not a driver mistake.
